Documentary Requirements
Interested and qualified applicants should signify their interest in writing. Attach the following documents to the application letter and send to the address below not later than JULY 6, 2022.
NON-LAWYER POSITION
- Application Letter
- Three (3) copies of duly accomplished Personal Data Sheet (CSC Form 212, Revised 2017) together with the Work Experience Sheet for all applicants, preferably typewritten;
- Authenticated CS Eligibility required for the position/Certified true copy of board rating (RA 1080) and Photocopy of updated PRC license;
- Certified True Copy of Transcript of Records (TOR);
- Photocopy of certificates of relevant trainings/seminars required for the position;
- NBI Clearance (For new applicants);
- Photocopy of Performance Evaluation/ IPCR (at least VS for the last two semesters immediately preceding the application (For promotion);
- For Non-Lawyer applying for the position of the Division Chief and Executive/Managerial Positions:
- Duly accomplished and signed Medical Certificate (CSC Form 211) with Psycho-Neurological Examination for Non-Lawyer applying for the position of Division Chief;
- Photocopy of Training Certificate of at least 40 hours of supervisory/management learning and development undertaken within the last 5 years for Division Chief;
- Photocopy of Training Certificate of at least 120 hours of supervisory/management learning and development undertaken within the last 5 years for Executive/Managerial Positions
LAWYER POSITION (Presidential Appointees)
- Application Letter;
- Three (3) copies of duly accomplished Personal Data Sheet (CSC Form 212, Revised 2017) together with the Work Experience Sheet for all applicants, preferably typewritten;
- Bar rating;
- Certificate of Admission to the bar;
- Clearances (Valid only for 6 months):
- Ombudsman Clearance
- Sandiganbayan Clearance
- Civil Service Clearance
- NBI Clearance
- Certification from the recommendee that he/she possesses all the qualifications and none of the disqualifications for the position;
- For recommendees with the pending case/s, to submit the following documents:
- Signed case brief/executive summary of the pending case/s by the concerned person or official who prepared the same;
- Recommendation letter from the Head of Agency affirming his/her recommendation despite pending case/s;
- Copies of affidavit complaint, with complete attachments/annexes;
- Counter affidavit; answer or reply with complete attachments/annexes;
- Rejoinder and comment to the rejoinder, with complete attachments/annexes (if filed);
- Other related documents, e.g., Office of the President/Sandiganbayan/Court of Appeals/Supreme Court decisions, if any.
- Photocopy of Performance Evaluation/IPCR (at least VS for the last two semesters immediately preceding the application;
- Original Sworn Statements, Liabilities and Networth (SALN);
- Duly accomplished and signed Medical Certificate (CSC Form 211) with required attachments from items 1-5.
LAWYER POSITION (Non-Presidential Appointees)
- Application Letter;
- Three (3) copies of duly accomplished Personal Data Sheet (CSC Form 212, Revised 2017) together with the Work Experience Sheet for all applicants, preferably typewritten;
- Bar rating;
- Certificate of Admission to the bar;
- Clearances (Valid only for 6 months):
- Ombudsman Clearance
- Sandiganbayan Clearance
- Civil Service Clearance
